We Help You Get the Most Out of Your Dental Benefits
At Timothy H. Kindt, DDS we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this, we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.
We are in-network providers for Aetna, Assurant, B.C.B.S. of AZ, Cigna, Delta Dental, LEVEL, Humana, United Concordia, and select United Healthcare plans. We would be more than happy to contact your insurance provider for you to find out how your benefits will work in our office. Check with your insurance company to be sure we are on your plan.
Please note: Medicare plans work differently; please contact our office directly for network status.
Because insurance policies vary, we can only estimate your coverage in good faith. This means we cannot guarantee your exact coverage due to the complexities of insurance contracts. Your estimated patient payment portion is due at time of service unless prior arrangements have been made. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment.
It is the patient’s responsibility to know and understand their insurance plan. However, we work diligently with you and your insurance provider for payment of services. We collect the ESTIMATED patient portion at the time of service based upon your insurance information received. If, for any reason, your insurance does not cover the remaining balance, a statement will be sent to the guarantor (patient or guardian financially responsible) for prompt payment of the remaining balance.
If you have any problems or questions, please call the office at (480) 939-5818
Please bring all your insurance information with you to the consultation so that we can expedite reimbursement.
